Your returning patient visit will be an online 1 hour review of your medical history and a Mind Body Spirit inventory since your last appointment. You and Dr. Wong will formulate a treatment plan together and decide how it will be executed.
If you have not had an appointment in a year or more click the "Update My Info" button below to confirm all your information, informed consent, and knowledge of any new clinic policies and procedures is up to date. Once you are updated in our system, you will be able to book your Returning Patient Visit online.
