I have not been seen in 1 year but I want to bypass the returning patient appointment and/or forms and proceed directly to treatment.
As per clinic policy all patients wanting to return to the clinic after not having an appointment in over 1 year are required to complete our returning patient intake forms and have a returning patient appointment.
This practice is to ensure ethical and effective medical practice and can not be bypassed. Our physicians are committed to conducting thorough assessments of patients who have not been seen within a year or longer.
